In a pure managed care plan, all medical services are provided by the managed care company either through
- Its own hospital or other health care facility or one with which it contracts
- Through doctors and other health care providers who are part of the company's network.
There is no bill -- so there is no bill to be concerned about filing.
There will be a claim to make if your plan permits you to obtain health care outside of the plan and the insurer pays part of the bill.
