Prioritizer

Among other uses, the chart can be used to keep track of questions you want to ask your doctor as you think about them. When you prepare for the meeting, you can number the questions numerically in your priority. You'll be sure to ask your most important questions. You can either e mail/fax/mail your doctor the remaining questions or ask when you can have another appointment.

When you are ready to prioritize your thoughts/questions:

• Count the number of thoughts and/or questions you entered.
• Number the thoughts and/or questions in priority. If you need to add additional items, click "Add Item."
• Click on "Save."

When you want to clear the chart, click "Clear."
