How To Obtain The Forms To Apply For Social Security Disability Insurance (SSDI)

It used to be that Social Security mailed you the forms to complete them before your initial interview with a Social Security representative. This gave you the advantage of being able to think about your answers, and to complete the forms in a way that gave you the best chance of succeeding.

The forms are no longer mailed to you because Social Security is moving to a paperless workplace. Instead, the forms are completed by the Social Security representative at your in-person or telephone meeting. However, you can still obtain the forms on line which we recommend you do to help you be better prepared.

You'll have time after setting the appointment to complete the forms.

We strongly suggest that you:

  • Obtain each form Social Security will use and
  • Complete the form before the meeting.

At least shorten the process and complete the worksheet before the meeting.

You can obtain a packet of forms (including the worksheet) by getting a copy of Social Security's Disability Starter Kit. Kits are available for adults and for children under age 18 at http://www.ssa.gov/disability/disability_starter_kits.htm offsite link
