  • A doctor of medicine or osteopathy authorized to practice medicine in the state.
  • Podiatrists
  • Dentists
  • Clinical psychologists
  • Optometrists
  • Chiropractors (limited to the treatment of manual manipulation of the spine to correct sublaxation that x-ray shows exists)
  • Nurse practitioners
  • Nurse-midwives
  • Physician assistant
  • Christian Science practitioners
  • Healthcare providers from whom employer's benefits plan accepts certification to pay benefits.
  • Foreign healthcare providers who practice in another country who are authorized to practice there and are practicing within the scope of his or her license.
