As you are aware, the determination of the existence of a health condition and its impact on your ability to live and work is made by a state agency generally known as DDS.
As part of the determination, DDS may send you or your health care provider a questionnaire about your health condition. There are no standard forms: they vary from state to state and from disease to disease.
When completing these forms:
- Be straight forward when answering.
- Try to tie the answers to the symptoms that are keeping you from working.
